RootBound Health is a direct-care clinic built for the patients the system discharges into instability — telehealth-first, after-hours staffed by clinicians, priced on a sliding scale. Opened November 2025 as a Direct Primary Care practice under RCW 48.150. Patients are accepted regardless of insurance, income, or ZIP code. The operating stack: Healthie EHR, Radix telehealth, 100Plus cellular RPM, and Cherry financing. Projected revenue is $2.8–3.4M in Year 1, scaling to $4.5–5.2M in Year 2, with operating break-even targeted at Month 8–12.

First hospital MOU, the PHSKC §318 conversation, the first grant filings, one rural pilot live.

By Day 90: every program has a named owner, the cadence runs without the founder, and no episode falls through — measured, not assumed.
